UMI Publishing Agreement and Submission Forms for Dissertations and Theses:
UMI publishes doctoral dissertations and master's theses authored by graduates of accredited institutions only, and only with a signed publishing agreement from each student/author. This page provides links to the forms and information needed to submit and publish dissertations and theses.
There is one document for Doctoral Dissertations and another for Master's Theses. These are identical except for the page border, which helps our staff quickly sort theses from dissertations in the acquisition process.

Please fill out the agreement form in its entirety. Any missing information will cause a delay in processing your thesis or dissertation. Print out the form for completion and send to ProQuest LLC, Dissertation Publishing Customer Service, 789 E. Eisenhower Parkway, P.O. Box 1346, Ann Arbor MI 48106, USA along with your manuscript.
